B2 Community Rehabilitation Assistant
Would you like o to be a member of this domiciliary rehabilitation team, providing short term, high-quality rehabilitation in a community environment. This will mean working without direct supervision carrying out tasks identified via personalised plans of care under the direction of the Nurses, Therapists and Associate Practitioners within the team.
The team strive to promote independence and recovery following discharge from hospital, or to prevent unnecessary hospital admission by providing the right patient with the care at the right time.
Delivering a compassionate, dignified and respectful service to patients at all times Ensuring the values outlined in the NHS constitution are adhered to daily and any matter of concern is raised with the relevant line manager or through the necessary processes within the Trust.
The candidate must have the following skills and experience:
- Car owner and full driving license
- Manual handling competent
- Experience of working in a Community setting
- Rehab experience desirable
- Knowledge of system one
- IPC trained
- Covid OLM completed
- No clinical vulnerability
- Medication management experience would be beneficial
