B6 Occupational Therapist or Social Worker
We are Looking for a band 6 Care Co-ordinators for long term contracts.
The post holder will work within the Adult Mental Health Team, providing mental health treatment in the community for patients with mental health needs, supporting patients to work towards their own recovery. The post holder will act as CPA Care Coordinator for a defined caseload. The post holder will also provide home treatment for patients experiencing a mental health crisis.
CVs will be accepted from RMNs, OTs or SWs.
Shifts Mon – Fri 9-5.
We will give consideration for 4 x 9.5 hour days rather than 5 x 7.5 hour days after a period of time.
Although some home working may be possible candidates will need to work on-site at least 2-3 days per week and must be prepared to travel.
