If you have empathy then this role is for you. Empathy goes a long way in helping to connect well with clients and handle their various needs with tenderness and warmth.You will work with adults, their family and carers who may require social care services, assessing their social care and health needs and to plan, implement, review and monitor action and outcomes.
Review and monitor each care plan and the quality of the service provision making changes in response to changing needs.
Full clean driver’s licence and access to a car (or able to use another appropriate means of travel) to travel to a range of locations including home visits.
