Practice Manager required for Community Active Recovery Team, Social Work Qualified position with Management experience essential. 

The role will support Qualified and Unqualified Social Workers in facilitating new assessments and care provisions for the residents. You will also support CHC Social Workers who assist in the Discharge to Assess model when individuals are no longer receiving funding from the CCG.

This senior role will manage the social care aspects of the Reablement Service. The social care staff will carry out assessments of social care needs and provision of support for vulnerable adults coming through the Reablement service to ensure they are safe and have good outcomes to promote independence. You will have in-depth experience of working in a busy Social Care Complex needs environment. 

This role is based in London.
