Social Worker

This role requires a professionally qualified Social Worker to work within a Generic Care Management Team. The candidate will provide recommendations to the Care Manager on the basis of assessed need and will implement Care Planning arrangements.

Under the direction of the Care Manger the candidate will undertake assessments of need and consider options and resources and make recommendation for care arrangements. To implement agreed care plans and to provide ongoing, direct Social Work Practitioner input as required and to reassess, review and evaluate and monitor care as required.

Essential requirements:

• Qualified Social worker registered with HCPC
• Care Act compliant Assessing skills & experience
• Ability to complete safeguarding adults enquiry 
• Risk assessing skills & experience
• MCA and Best interest decision making skills & experience
• The social work interventions will focus on investigating safeguarding Adults concerns. 
• Driver with access to a car
