As a practice manager, you will be a part of a team of up to 4 workers to work alongside colleagues in integrated services.
You will work with the existing teams to identify and ensure support plans are in place.
To oversee, undertake and ensure a Care Act and or s117 assessment and/or review of those in priority placements and all NRPF cases takes place.
You will have a target of 6 reviews/reassessments per week which has been set for each worker.
